Notre Dame Hospice Care provides comfort, compassion, companionship, and support to patients nearing the end of life and their family members when a treatment or cure for an underlying disease is no longer available. Services are delivered by a dedicated team of health care professionals who focus on addressing physical, mental, social and spiritual needs to improve quality of life.

At Notre Dame Hospice, plans of care are individualized to enable each patient to navigate through the end of life with hope and meaning while managing symptoms and reaching their goals of care.

In line with Notre Dame Health Care’s mission of embracing a holistic approach to care for all the people we serve, our team members not only assess and manage all physical symptoms related to their specific illness and end-of-life care, but also the emotional, spiritual and social aspects of coping with a terminal illness.

Our Hospice doctors also pay special attention to honoring the specific needs and customs of all ethnic backgrounds, celebrating each individual through every stage of their home care.

What does Hospice Care provide?

We know that if you’re unfamiliar with home Hospice Care, you might not know what is provided. While each program is different and can vary by community or location, we’re pleased to share that Notre Dame Hospice Care provides:

  • Physician, nurse practitioner, and nursing at home
  • Social services
  • Assistance in living with a terminal diagnosis and navigating the complexities of end-of-life decisions
  • Resources and education on advance directives, Medicaid applications, and more
  • Medications related to terminal diagnosis
  • Medical equipment for symptom management and pain relief
  • Aide and homemaker services

  • Other covered services you need to manage your pain and other symptoms
  • Spiritual and grief counseling for you and your family members
  • 24-hour-a-day, seven-day-a-week on-call availability of a qualified hospice professional

Our team members offer on-call availability 24 hours a day to provide comfort and peace of mind to the patient and their family.

Who makes up the Hospice Team?

The Hospice Team at Notre Dame Health Care is made up of a dedicated group of various interdisciplinary professionals. This can include:

  • Medical directors who oversee the medical care provided to hospice patients
  • Registered nurses who assess patient needs, manage symptoms and offer emotional support to patients and families
  • Social workers who help patients and loved ones navigate end-of-life-care
  • Spiritual care that provides spiritual and emotional support for families
  • Home health aides who provide personal care and assistance with activities of daily living
  • Massage therapists who provide pain relief and relaxation
  • Music therapists who use music interventions to address physical, emotional, cognitive and social needs of patients
  • Volunteer coordinator and trained volunteers who offer companionship and support
  • Pharmacists who collaborate with health care teams to ensure effective medication management
  • Bereavement care specialists who provide support to families and loved ones after a patient’s death, helping them cope with grief and loss
