Led by our team of experienced therapists, each patient within our Short-Term and Long-Term Rehab Center receives a customized treatment plan tailored specifically to their needs.
Therapy services are located in a dedicated space to facilitate recovery after a hospital stay or injury. Whether short- or long term care is needed, Notre Dame Rehab Center offers:
Short-Stay Rehab patients receive individualized, custom care, with six days of therapy a week, as needed, allowing the individual to return home as soon as possible. Short-Stay Rehab also offers the same services and amenities that our Long Term Rehab residents enjoy, including delicious cuisine and therapeutic activities.
Notre Dame Health Care’s Long Term Rehab services provide extended care and Rehabilitation for individuals recovering from serious illnesses, surgeries, or chronic conditions.
Unlike Short-Stay Rehab, which typically lasts a few weeks, our Long Term Rehab is designed for those who need continual therapy and care as needed during their stay. During this time, our goal is to help individuals regain as much independence as possible while managing their needs in a supportive environment.
Short-Stay and Long Term Rehabilitation services are provided at Notre Dame Health Care by our team of skilled, experienced in-house therapists. Our team excels at thinking outside the box to ease the concerns of individuals with a variety of health issues and care needs through the Rehabilitation process.
Our interdisciplinary team works with both patients and families to create a discharge plan that will ensure and facilitate a workable, safe and easy transition for individuals to safely return to their home setting and an enhanced quality of life.

Yes, we are able to provide Short-Term Rehabilitation five to six days a week. Please have your hospital case manager make a referral to Notre Dame Health Care.

This can be entirely customized to meet resident and patient needs. Typically, days will be structured around meals, various types of therapies, care, enriching programs, and times for rest and recovery.
Those within our Rehabilitation program enjoy the same services and amenities that residents have access to each day.
A smooth transition home depends on a number of factors. To ensure a safe transition, we’ll assess physical, cognitive, emotional and medical readiness. We may also assess whether or not their home is ready while determining a plan for care at home. Check-ins and follow-ups may also be conducted.
